Our professional team provides the quality care your child needs and the advice and counsel you want. Caring for children from birth through adolescence is a great privilege and a responsibility that we enjoy and take seriously. An essential part of our work is to educate and empower our patients and their parents and families to lead healthy lives. For our patients who live with chronic illness, physical or emotional disabilities or developmental delays, we work closely with their specialists to coordinate their care. We view our relationship as a partnership in which we work with families to assure that each and every child and teen can achieve his or her fullest potential.

We are happy to provide our office forms for convenient download on your home computer. Please print and complete the forms below, and bring them with you to your child's first appointment. Questions about which forms you need? Please call our office and our courteous staff will assist you in preparing for your child's first visit.
Insurance and Billing
We accept the following insurance companies:
- Blue Cross/Blue Shield
- Keystone CHiP
- UPMC Healthplan
- UPMC for Kids (CHiP)
- UPMC for You (Medicaid)
- Cigna
- Health America
- United Healthcare
We will submit claims on your behalf for other insurance companies that we do not participate with, however the claim balance will be your responsibility to pay.
Payment Options
We accept checks, cash or credit cards (Mastercard, Visa and Discover). We also offer a flexible payment plan. Please see our Financial Coordinator for details.
