 The patient portal will allow you to communicate electronically with our staff and physicians.  It will also provide other services that can be accessed at your convenience.  The next time you are in our office for a visit, ask about getting set up to use the portal!

EFFECTIVE APRIL 1, 2021, THE 21ST CENTURY CURES ACT, NEW FEDERAL AND STATE CONFIDENTIALITY LAWS AND HIPAA, IS MAKING IT MANDATORY FOR US TO CHANGE THE WAY WE CAN PROVIDE MEDICAL INFORMATION TO PARENTS THROUGH OUR PATIENT PORTAL!

PORTAL INFORMATION REGARDING MINORS AGE 0-13

One "Authorized Representative" portal account will be given to a parent/legal guardian of children age 0-13.  The day before the patient (child) turns 14, the portal will be deactivated.  You can download and print the form below, fax, mail or bring to our office and then we can provide you with an access code.

PORTAL INFORMATION REGARDING MINORS AGE 14-17

Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.  This includes information regarding the screening and treatment for drug and alcohol related issues, mental health, reproductive health and certain diseases.    For this reason, it is necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative.  Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor’s patient portal. This consent will remain in effect for one (1) year unless the child (patient) revokes in writing.

***The above confidential information will not be disclosed to a parent/guardian without the child's consent, unless it is in response to an emergency medical situation when the release of the information is necessary to prevent serious risk of bodily harm or death.

PORTAL INFORMATION REGARDING MINORS AGE 18 AND ABOVE

At age 18, the patient is considered an adult.  The portal will be deactivated and only the patient will be permitted to create a portal account.

What is a Portal?

WHAT IS A PATIENT PORTAL?

The patient portal is designed to enhance patient - provider communication. The secure web portal is a way to view certain health information for your child and communicate non-urgent information with our staff. Meadville Pediatrics provides this MEDENT Patient Portal for the exclusive use of its established patients. The patient portal is designed to enhance patient - provider communication. 

 Some of the features offered with this service include:

  • Ability to view patient demographic information and send requests to our staff to update this information. If you have access to more than one child’s account, you will need to make updates for each patient individually
  • Medication lists and the ability to request refills
  • Immunization Records
  • Messaging with your provider’s medical staff for non-urgent questions

 HOW DO I SIGN UP FOR THE PORTAL?

Once we have a signed consent form on record, we will provide you with an access code and instructions to create an Authorized Representative portal account; follow the login instructions in the letter and input your name exactly as it appears on the letter we provide you (even if we have the incorrect spelling!)  The  consent forms are available to be signed ONLY at our office.  For parents wishing to have portal access for their children ages 14-17, both the patient  (child) and parent must be present at the office to sign the consent.

PATIENT PORTAL IS NOT INTENDED FOR THE FOLLOWING:

  • Diagnosis or extended treatment; all advice given over the patient portal is for routine inquiries that are nonurgent in nature. Should the medical team need additional information, they will reach out to find a time for a phone follow up. All response via the patient portal is at the discretion of the physician and his or her staff. They reserve the right to require an in-person visit to provide diagnosis/ treatment.
  • Emergency communication. Communication on the portal is restricted to non-urgent issues. If your child is experiencing an emergency, please dial 9-1-1 or go to the nearest emergency room.

PROTECTING YOUR CHILD’S PRIVATE HEALTH INFORMATION AND RISKS:

While we work hard to ensure that all communication through the portal is secure, it is imperative that Meadville Pediatrics has your correct email address and that you inform us of any changes to your email. It is your responsibility to make sure your login information is protected from unauthorized persons. If you think someone has learned your password, please promptly change it or call our office. Your email address is confidential and protected information, Meadville Pediatrics will never purposefully share this information with a third party.

ACCESS FOR CHILDREN 14 -17 YEARS OLD:

According to Pennsylvania law (2004 Act 147, 55 PA Code 5100.33), minors 14 years of age or older can give consent to treatment without parent consent, and as such, we need to extend confidentiality to those patients as we would an adult. For this reason, patients 14 and older who wish their parent to have access to the Patient Portal will be asked to sign a release form granting their responsible party access to this information. Without a signed release form, access to the portal account will be disabled for responsible parties when the patient turns 14.

WHAT INFORMATION WILL I BE ABLE TO ACCESS IN MY CHILD’S PATIENT PORTAL ACCOUNT?

BIRTH TO AGE 14

One authorized representative (in most cases, one of the parents/legal guardians) will have full access to all chart information that is available through the Cares Act.  This includes upcoming and past appointment records, immunization records, growth charts, allergies, current medications, demographic information, referral to other specialists, visit summaries, health history, problem list, and lab and x-ray results.  You will be able to send messages to the providers.  One day before your child turns 14, the portal account will be deactivated.

**For our patients age 11-13:  there may be confidential information in the chart that will not be available for the parent/legal guardian to have access to.  This information may be, but is not limited to: drug/alcohol screening results, drug/alcohol treatment, sexually transmitted diseases, pregnancy, psychiatric/psychological/behavioral care and/or confidential HIV related information.

AGE 14 to 18

If your child gives consent for you to have access to the patient portal, he/she must sign a consent, giving you the permission.  This will be good for 1 year and may be revoked at any time at the request of the child (patient).   With consent given, you will have access to everything listed above. 

***Please note that even with consent, there may be confidential information in the chart that will not be available for the parent/legal guardian to have access to.  This information may be, but is not limited to: drug/alcohol screening results, drug/alcohol treatment, sexually transmitted diseases, pregnancy, psychiatric/psychological/behavioral care and/or confidential HIV related information.

AGE 18 and above

Only the patient (18-year-old) will have the option to have a portal.  We only see 18-year-old patients until they graduate from High School, so as soon as they transfer to another provider, the Meadville Pediatrics patient portal will be terminated permanently.  There will not be an option for an Authorized Representative for any patient age 18 and above.

 **Please note, this does not impact how you receive information and communicate with your child's health care providers outside of the portal. Parents/guardians may communicate with Meadville Pediatrics providers and staff as you always have until your child reaches age 18.  However, any confidential information will not be disclosed to a parent/guardian without the child’s consent, unless it is in response to an emergency medical situation when the release of information is necessary to prevent serious risk of bodily harm or death.
